nPOSITION SUMMARY
nThe Case Manager serves as the primary client-centered service professional responsible for guiding families from homelessness to housing stability and long-term independence. This position provides trauma-informed, goal-driven case management services that address housing, employment, education, financial stability, childcare, transportation, and behavioral health barriers.
nThe Case Manager develops individualized service plans, monitors measurable progress, and ensures compliance with HUD, Continuum of Care (CoC), and Providence House standards. Through structured accountability, coordinated referrals, and consistent follow-up, this role directly supports sustainable housing outcomes and family self-sufficiency.

nKEY RESPONSIBILITIES
nClient Case Management
nConduct comprehensive intake assessments to identify strengths, barriers, and service needs.
nDevelop individualized case plans with measurable goals related to housing stability, employment, education, and family well-being.
nMeet with clients regularly (minimum bi-weekly or as needed) to monitor progress and adjust goals.
nProvide trauma-informed support, motivational coaching, and accountability guidance.
nMaintain manageable caseloads assigned by the Director of Housing.
nDocument all services, case notes, and progress updates in HMIS and internal systems within required timelines.
nCoordinate referrals to community partners including healthcare, financial counseling, legal aid, workforce development, and childcare resources.
nFacilitate life skills, financial literacy, and housing readiness workshops as assigned.
nHousing Stability & Transition Support
nCollaborate with the Housing Team to identify appropriate housing placements.
nAssist clients with housing applications, landlord communication, lease preparation, and move-in planning.
nConduct home visits post-placement to monitor stability and identify emerging needs.
nMaintain structured follow-up for 90 days post-exit to support long-term housing retention.
nTravel throughout the community to conduct home visits, attend partner meetings, accompany clients when appropriate, and provide supportive services related to housing stability and self-sufficiency.
nData Management & Compliance
nEnter and update client information in HMIS with accuracy and confidentiality.
nEnsure all documentation complies with HUD, CoC, and Providence House requirements.
nParticipate in internal audits, data quality reviews, and compliance monitoring.
nSubmit client outcome data to support grant reporting and performance measurement.
nCollaboration & Professional Accountability
nParticipate in case conferencing, multidisciplinary team meetings, and staff development sessions.
nCommunicate effectively with Directors and cross-department staff to provide coordinated wraparound services.
nAdvocate for clients while maintaining professional boundaries and ethical standards.
nSupport internal and external audits with accurate and timely documentation.
nMaintain strict confidentiality in accordance with HIPAA and agency policies.
nEducation and Experience
nBachelor’s degree in social work, Human Services, Psychology, or related field preferred.
nMinimum of two (2) years of experience in case management, housing services, or related human
nFamiliarity with HMIS and HUD/CoC program requirements strongly preferred.
nExperience working with families experiencing homelessness or poverty-related barriers preferred.
nValid driver's license required. Candidates without a current valid driver's license must demonstrate an active plan to obtain or reinstate driving privileges within a timeframe approved by Providence House.
nMust maintain driving privileges and remain insurable under Providence House's automobile insurance requirements throughout employment.
