Case Manager
LHH seeks a dedicated Case Manager. In this role, you'll play a pivotal part in supporting individuals and families as they navigate complex challenges. Whether it's providing one-on-one guidance, ensuring comprehensive care, or connecting clients to essential resources, your contributions will have a direct and meaningful impact.
What You'll Do:
- Complete progress notes on every face-to-face/telephone contact with participants or collateral contact by the end of each shift.
- Provide facility tours to guests and new program participants.
- Provide support to the case managers and program participants to support guest goals as established in individualized service plans.
- Monitor and maintain the interim housing floors to ensure a hospitable and inclusive environment. This may include keeping program participant space organized and clean, providing hygiene needs, etc.
- Work in collaboration with the case management and operations team to ensure that the facility is clean and safe for all.
- Provide crisis intervention services focused on enhancing the participants ability to independently problem solve, use effective coping skills, and manage own care.
- Use evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, and Harm Reduction.
- Utilize lived experience to cultivate strong rapport and engagement in services.
- Provide interim housing guests with information, referrals, linkages, and advocacy for identified needs as much as possible.
- Creatively use and develop community resources to broker and link participants to services.
What We're Looking For:
- Bachelor's degree in Social Work or related field (preferred but not required)
- A minimum of 1+ year of experience with mental health or homelessness (volunteer experience counts!!)
- Valid driver's license, reliable transportation, and TB clearance
Location: San Jose, CA 95121 onsite
Swing Shift 1p-9:30p (Sun-Thurs or Tues-Sat); OR Sun-Thurs Day Shift 8a-5p
Pay Details: $25.00 to $31.00 per hour
