nJob Summary
nThe Case Manager provides harm-reduction, trauma-informed, and evidence-based care coordination services within a client-centered framework. This position supports clients in achieving recovery goals, improving functional outcomes, and sustaining engagement through individualized case management, psychoeducation, and relapse prevention planning.
nThe Case Manager also collaborates closely with multidisciplinary team members and external partners—including county agencies and private/commercial insurance representatives—to ensure continuity of care, accurate documentation, and compliance with authorization requirements.
nThis role is vital to promoting empowerment, client engagement, and long-term success in recovery and reintegration.
nEssential Duties and Responsibilities
nSupport completion of enrollment, consents, and other required documentation.
nFacilitate individual and group psychoeducational sessions focused on recovery skills, coping strategies, and relapse prevention.
nConduct comprehensive case management assessments and develop individualized service plans aligned with client treatment goals and problem lists.
nProvide ongoing coordination of services, linking clients with appropriate community resources, healthcare providers, and support systems.
nMaintain accurate, timely, and compliant progress notes and documentation within the electronic health record.
nConduct internal chart reviews, identify missing or incomplete information, and ensure timely corrections.
nPrepare and issue enrollment, progress, or completion letters as needed.
nCollaborate with clinical, medical, and administrative staff to ensure alignment with treatment and discharge planning.
nComply with all program rules, policies, and procedures—including HIPAA and other regulatory standards.
nComplete all required trainings and maintain compliance with county contract and credentialing standards.
nMinimum Qualifications
nCertification by one of the following California credentialing bodies:
nCalifornia Association of DUI Treatment Programs (CADTP)
nCalifornia Consortium of Addiction Programs and Professionals (CCAPP)
nCalifornia Association for Alcohol/Drug Educators (CAADE)
nValid & current California Driver’s License
nValid & current CPR/First Aid Certification
nValid & current TB clearance
nEmployment health exam clearance
