- JOB SUMMARY
- The Case Manager role is defined as a collaborative process used to assess, plan, implement, coordinate, monitor, and evaluate options and services to meet individuals' health needs through communication and available resources to promote quality, cost-effective outcomes.
- JOB QUALIFICATIONS
- Education:
- Registered Nurse with an AD, BSN, master's degree, or doctoral degree
- Social Worker with a bachelor's degree in social work, master's degree, or Doctoral degree in Social work
- Certifications/Licenses:
- Registered Nurse with a license to practice in Wyoming.
LCSW with a license to practice in Wyoming
- Registered Nurse with a license to practice in Wyoming.
- Job Knowledge and Skills:
- Five years' clinical experience
- Two years' experience in case management/utilization management preferred.
- InterQual application preferred
- Education:
Case Manager in rock springs at Unknown Company
