Case Manager RN
Location: Waukesha, WI 53189 Schedule: 8:00 AM - 4:30 PM, weekdays + one weekend every 6 weeks Employment Type: Full-time Dress Code: Business casual
Pay Rate: $40-$44/hr
About the Role
The Case Manager (CM) is responsible for continuum of care planning for complex hospital patients, ensuring safe, timely, and appropriate transitions to the next level of care. This role requires strong clinical judgment, patient advocacy, and collaboration with interdisciplinary teams.
Key Responsibilities
Daily Responsibilities

  • Comprehensive discharge planning
    • Assess patient needs for post-hospital services and determine appropriate discharge plans for complex cases.
    • Identify barriers to discharge and develop mitigation strategies.
  • Patient & family education
    • Educate patients and families on available care options within the appropriate level of care.
    • Provide guidance on expectations, insurance considerations, and transition logistics.
  • Interdisciplinary collaboration
    • Work closely with physicians, nurses, therapists, clinical social workers, and contracted payor case managers.
    • Participate in care rounds and coordinate early involvement of next-level-of-care providers to ensure continuity.
  • Documentation & order management
    • Initiate and complete all discharge-related paperwork.
    • Facilitate completion of required physician orders prior to transfer to avoid delays.
    • Ensure documentation meets CMS and Joint Commission standards.
  • Care transition coordination
    • Communicate all relevant clinical and social information to receiving facilities or providers.
    • Arrange transportation, equipment, and services as needed.
    • Ensure smooth handoff and continuity of care.
  • Regulatory compliance
    • Adhere to CMS, Joint Commission, and hospital policies.
    • Maintain accurate, timely documentation aligned with regulatory expectations.
Required Qualifications
  • Associate Degree in Nursing (ADN)
  • Bachelor of Science in Nursing (BSN) preferred
  • MSN strongly preferred
  • Licensed Registered Nurse (RN) in Wisconsin
Shift & Schedule
  • Hours: 8:00 AM - 4:30 PM
  • Days: Primarily weekdays
  • Weekend rotation: One weekend every 6 weeks
