Case Manager - RN/LSW: Discharge & Care Coordination

Firelands Health is seeking a Case Manager to oversee the Case Management process, monitor physician documentation, and coordinate utilization reviews and discharge planning with interdisciplinary staff. The role requires RN and social work credentials and several years of acute care experience.

The ideal candidate will work with the Social Worker and medical staff to ensure appropriate hospitalization, timely discharge, and referrals to post‑acute services. Ohio licensure and LISW preferred.
