Join our healthcare team as a Case Manager and play a pivotal role in patient care coordination. This full-time role is ideal for experienced Registered Nurses (RN) and Licensed Master Social Workers (LMSW, LISW, LCSW) who are committed to advocating for patients and ensuring a seamless healthcare experience.
Key Responsibilities:

• Conduct assessments to determine patient needs and create safe discharge plans
• Coordinate with post-acute care providers, including home health, rehab, and skilled nursing
• Collaborate with healthcare teams to ensure high-quality care and smooth patient transitions
• Serve as a liaison for families, healthcare providers, insurance, and community resources
• Maintain compliance with care coordination standards, documentation, and patient education

What We're Looking For:

• Education & Licensure:
  • RN: Associate or Bachelor's Degree in Nursing from an accredited program
  • LMSW/LISW/LCSW: Master's Degree in Social Work
  • Active RN or Social Work license required
  • BLS certification required
  • Case management certification (ACM, CCM, RN-BC) is a plus
• Experience & Skills:
  • At least 2 years in healthcare, preferably in an acute care environment
  • Experience with discharge planning, case documentation, and regulatory compliance
  • Strong communication, problem-solving, and critical thinking abilities
  • Familiarity with EMR systems and case management software preferred
