RN Care Coordinator - Full-Time
Schedule: Monday-Friday, 8:00 AM-5:00 PM Role Type: Onsite, outpatient clinical setting
About the Role
The RN Care Coordinator plays a key role in managing patient care plans, triaging clinical needs, and ensuring seamless communication across the care team. This position requires strong clinical judgment, excellent patient-education skills, and the ability to coordinate complex care in a fast-paced environment.

- Care Plan Coordination - Coordinate patient care plans across providers, departments, and services.
- Clinical Triage - Perform triage via phone, EMR messaging, or in person.
- Result Review - Review labs, imaging, and clinical results; escalate findings appropriately.
- Procedure Preparation - Prepare patients for procedures and ensure all pre-op requirements are met.
- Patient Education - Educate patients on treatment plans, medications, and follow-up care.
- Referral & Authorization Management - Manage referrals and authorizations, especially in outpatient settings.
- EMR Documentation - Document all clinical interactions accurately and in a timely manner.
- Care Team Communication - Serve as the primary point of contact between patients, families, and the care team.

- RN Experience - 3-5+ years RN experience.
- Family Medicine Background - Experience in family medicine preferred.
- Patient Education Skills - Strong patient-education and care-coordination abilities.
- EMR Proficiency - Epic experience strongly preferred.
- Multidisciplinary Teamwork - Comfortable working across multiple clinical teams.
- High-Volume Care Management - Ability to manage high patient volumes and complex cases.
- Active RN License - Required.
- Clinical RN Background - 2-3+ years of clinical RN experience.
- Care Coordination Experience - 1+ year preferred.
- Clinical Triage Skills - Strong triage and assessment capabilities.
- Communication Skills - Excellent communication and patient-education skills.
- Fast-Paced Environment - Ability to manage multiple priorities efficiently.
