Our client is currently seeking a Case Manager - Registered Nurse

Schedule: Monday-Friday | 8:00 AM-4:30 PM

About The Opportunity

We are seeking an experienced Registered Nurse (RN) Case Manager to join a collaborative healthcare team in an acute hospital setting. This role is ideal for an RN who enjoys working directly with patients, coordinating care across disciplines, and helping ensure patients receive the right level of care while supporting safe and timely discharges.

Key Responsibilities

  • Conducting admission and ongoing case management assessments to evaluate medical necessity, level of care, and progression of treatment.
  • Reviewing patient cases to ensure appropriate utilization of hospital resources and compliance with regulatory requirements.
  • Collaborating daily with physicians, nurses, social workers, and other interdisciplinary team members to develop and execute patient care plans.
  • Identifying and addressing barriers to discharge, helping patients transition safely to the next level of care.
  • Coordinating post-acute services, community resources, and placement options to support continuity of care.
  • Facilitating proactive discharge planning to optimize patient length of stay and reduce avoidable delays.
  • Completing timely admission reviews, continued stay reviews, and required CMS documentation, including IMM and MOON forms.
  • Contributing to efforts aimed at reducing readmissions and improving patient outcomes.

Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license.
  • Recent Case Management experience in an acute hospital setting.
  • Strong patient-facing/bedside nursing experience.
  • Experience using Cerner EHR.
  • Excellent communication, critical thinking, and care coordination skills.
  • Ability to work effectively within a multidisciplinary healthcare team.

Preferred Skills

  • Strong organizational and time-management abilities.
  • Knowledge of utilization review, discharge planning, and care transition processes.
  • Experience working with complex patient populations and high-risk cases.
