Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate evaluations, develop client-focused care plans, and ensure access to necessary resources while working remotely. Key responsibilities: Coordinate with patients and caregivers to assess needs, goals, and current services, determining eligibility and benefits for admissions Develop and communicate comprehensive case management plans in collaboration with healthcare teams, patients, and caregivers Monitor patient progress towards treatment goals, identifying barriers and ensuring adherence to regulatory guidelines Required qualifications: Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field Experience in coordinating resources and services to ensure continuity and quality of care Knowledge of regulatory, accreditation, and regional guidelines related to case management Ability to document patient cases accurately in medical files Understanding of the healthcare industry and patient disposition planning processes
