Managed Care organization seeking Care Managers to support Medicare and MLTC members. This is a great opportunity for an experienced RN or MSW with care management/case management experience who is bilingual and enjoys coordinating services and advocating for members.
💻 Hybrid Schedule:
• First 90 days: Tuesday–Thursday in office; Monday & Friday remote
• First week: Monday–Thursday in office
• Ongoing: 3 days in office / 2 days remote
• Hours: Monday–Friday, 9:00 AM–5:00 PM
🗣️ Bilingual candidates needed in:
Chinese | Spanish | Bengali | Urdu | Gujarati
What you'll do:
• Serve as a primary point of contact for health plan members
• Conduct monthly member outreach and follow-up
• Coordinate care with members, families, and vendor agencies
• Review and support person-centered service plans
• Identify and address member needs
• Coordinate covered and non-covered services
• Maintain accurate and timely documentation
• Participate in weekly team meetings
Caseload:
• Medicare: approximately 100–120 members
• MLTC: approximately 130–150 members
Qualifications:
✔️ MSW or RN required
✔️ Case management/care management experience or strong knowledge of case management
✔️ Experience with Medicare and/or MLTC populations preferred
✔️ Bilingual in one of the languages listed above
✔️ Strong communication, organization, and documentation skills
✔️ Comfortable managing a high-volume caseload
