Case Manager Registered Nurse – Long‑Term Services and Supports (LTSS)
Field‑based position out of Lake County, IL.
- Coordinate all case management activities with members to evaluate medical needs.
- Facilitate member’s overall wellness.
- Develop a proactive course of action to address issues.
Requirements
- Minimum 2+ years of clinical experience required.
- Active and unrestricted RN license in the state of IL.
- Must reside in Lake County IL.
- Must possess reliable transportation and be willing and able to travel up to 50–75% of the time.
- Case management and discharge planning experience preferred.
- Managed Care experience preferred.
- Microsoft Office Suites experience preferred.
- Minimum of Associate’s Degree in Nursing required.
- Bachelor’s Degree in Science and Nursing preferred.
