Case Manager / PRN ( RN / RT / SW / LPN )

Position at Select Specialty Hospital, a Critical Illness Recovery Hospital (LTACH) in Florida. Requires current licensure in a clinical discipline – nurse (RN, LPN, LVN), respiratory therapist, or licensed social work professional. Shift availability: minimum 2 shifts per month, weekdays approximately 8 a.m.–5 p.m., on an "as needed" basis. Prior discharge planning experience preferred.

Responsibilities

  • Develop and implement patient‑specific, safe and timely discharge plans.
  • Perform verification of utilization criteria reviews.
  • Coordinate with payor sources to assure proper reimbursement and promote cost‑attentive care through resource management.
  • Demonstrate compliance with utilization management policies and procedures.
  • Coordinate UR compliance with Quality Management to ensure licensure and accrediting requirements are met.
  • Maintain fiscal responsibilities, identify and negotiate reimbursement to maximize insurance benefit coverage, review insurance verification forms to minimize risk.
  • Facilitate multidisciplinary team meetings with physicians, nurses, respiratory therapists, and rehabilitation therapists.
  • Complete medical record documentation.
  • Report directly to the Director of Case Management and provide social work services as required per state guidelines.

Qualifications

  • Current licensure in a clinical discipline: RN, LPN, LVN, respiratory therapist, or licensed social work professional.
  • Previous inpatient hospital experience with critical/acute care patients (e.g., ICU, step‑down, med‑surg, ventilator support).
  • Experience in an acute medical case management setting and confidence to manage and direct care plans for chronically critically ill populations.

Preferred Qualifications

  • Specific experience in care management and discharge planning.
  • Working knowledge of the insurance industry and government reimbursement.
  • Availability for at least 2 shifts per month on an "as needed" basis, weekdays 8 a.m.–5 p.m.
