Case Manager - Path Program

Are you passionate about helping individuals experiencing homelessness, serious mental illness, and housing instability build a path toward recovery and independence? Join the Cumberland County Guidance Center as a Full-Time Case Manager with our PATH (Projects for Assistance in Transitioning from Homelessness) Program.

In this rewarding community-based role, you'll help connect individuals to housing, mental health services, healthcare, public benefits, and community resources while serving as a trusted advocate and support system. If you have experience in case management, behavioral health, social services, community outreach, or human services, this is an opportunity for you to make a meaningful impact every day.

What You'll Do

As a PATH Case Manager, you'll work directly with individuals experiencing homelessness and serious mental illness to help them obtain stable housing and supportive services.

Key responsibilities include:

  • Conducting community outreach and engagement activities
  • Developing Individual Recovery Plans (IRPs)
  • Providing ongoing case management and care coordination
  • Assisting clients with permanent, supportive, and emergency housing resources
  • Connecting individuals with mental health, medical, substance use, vocational, and educational services
  • Assisting with public benefits and entitlement programs
  • Utilizing Motivational Interviewing (MI) and recovery-oriented interventions
  • Maintaining accurate clinical documentation and compliance requirements
  • Supporting recovery and long-term independence

What You'll Need
Required Qualifications
  • Bachelor's Degree in Social Work or a related Human Services field
  • Minimum two years of experience in fields such as:
    • Case Management
    • Behavioral Health
    • Mental Health Services
    • Social Services
    • Homeless Services
    • Human Services
    • Community Outreach
  • Knowledge of housing resources and public benefit programs
  • Strong communication, advocacy, and relationship-building skills
  • Valid driver's license with acceptable driving record
Preferred Qualifications
  • Experience working with individuals experiencing homelessness or housing instability
  • Experience supporting individuals with serious mental illness or co-occurring disorders
  • Knowledge of Housing First and supportive housing models
  • Experience with crisis intervention and care coordination
  • Bilingual English/Spanish
  • Experience utilizing Motivational Interviewing and trauma-informed care
