This program serves youth and their current caregivers due to the incarceration of the youth's parent(s). It is a prevention and supplemental behavioral health program that collaborates with a multitude of community resources with the aim of providing stability and improving functioning of family members. Services also provide the caregivers opportunities to enhance their parenting strategies.

Responsibilities:

  • Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
  • Work with children, caregivers, and parents to address unmet needs and support them as they navigate through their barriers.
  • Provide direct service either in the office, via telehealth or in the field/homes.
  • Initiate and coordinate interventions, review and monitor progress, and reassess needs to ensure successful outcomes.
  • Assist clients in developing formal and informal community supports, while partnering with local agencies and resources.
  • Provide advocacy on behalf of clients.
  • Prepare and complete all required case recordings in an electronic medical record, administrative reports, statistical reports and related paperwork or database records in a timely and professional manner.
  • Attend clinical and administrative staff meetings, and multi-discipline team meetings as required.
  • Review treatment plans and progress of cases with supervisor for continued support, guidance and approval.
  • Perform other duties as necessary to accomplish objectives.

Education
Preferred

Bachelors or better in Social Work or related field.

Experience
Required
1-2 years:

Experience working with prison/jail population as families have a parent with incarceration.

1-2 years:

Experience working in homes, in community, and in office in a flexible way for client needs and maximum impact.

1-2 years:

Case management experience or formal training with knowledge of local community resources. Able to provide community outreach to represent and grow agency and program services.

1-2 years:

Able to work in a self-directed manner to effectively manage cases, accomplish required documentation and report on outcomes in a timely manner, utilizing excellent time management skills.
