Must-Have: - Oncology or Surgical Oncology Experience - Care Coordination Experience
Description: Schedule: Monday to Friday. Focus on breast cancer screening and care coordination for preoperative patients. The majority of patients will require assistance in preparation for surgical procedures. Data Abstraction is a key component of the role. Required to conduct follow-ups with patients diagnosed with cancer, general hematology, and oncology. Special procedures include simple dressing changes and occasional blood draws. Candidates must possess a background in managing complex care needs and coordinating patient services. The role requires a strong understanding of admission criteria and medical necessity per evidence-based guidelines. The position will include daily patient assessments and the development of a comprehensive plan of care. Collaboration with interdisciplinary support services will be essential during patient interactions. Experience with benefits eligibility, prior authorizations, and surgical services is mandatory. Skills in disease management and needs assessment/order DME are advantageous.
