RN Case Manager For Hospital Care Coordination-Discharge Planning

Position Duties:
- EMR: EPIC
- InterQual criteria or Milliman experience: InterQual
- Position for UR, discharge planning, or both: Acute Care Coordination (Inpatient nursing units, potential for ED and Maternity coverage)
- What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Inpatient nursing units, potential for ED and Maternity coverage
Experience or 'Must have' skills: MCG/InterQual reviews, Acute Care Coordination discharge planning experience
Shifts & Scheduling:
- Do you allow self-scheduling? Block scheduling: Mon-Fri 8-1630
- Weekend Requirements: No
- Holiday Requirements: No
- How many shifts of unit/department specific orientation?: 3-5 days
Additional Notes: Fast paced turn around for discharging patients. Attributes that will help someone be successful in this position include communication, time management, organizational skills, leadership, adaptability, problem solving
Minimum Required Qualifications: BLS Certification, Epic & hospital care coordination-discharge planning experience needed
Length of Assignment: 13 weeks
Shift / Hours per Week: Mon-Fri hours per week
