nBaccalaureate degree in Social Work, Psychology, Sociology, or related field with 2+ years of direct social service experience
nAssociate degree in Human Service, or related field with 4+ years of direct social service experience
nHigh School Diploma/General Equivalent Diploma with 8 plus years of direct social service experience
nStrong interpersonal skills with the ability to be compassionate and firm and always maintaining confidentiality
nKnowledge of community resources
nAbility to solve problems, make decisions, resolve conflicts and listen
nFluent in English and Spanish is a plus

nResponsibilities
nThe successful candidate will provide a full range of on-site, person-centered, strengths-based services to a caseload of approximately 20-25 clients with the objective to help people live fulfilling lives in the community
nThe Case Manager will be responsible for counseling, client intakes, case management, and counseling and aftercare services
nThe manager will collaborate with colleagues to meet the needs of their caseload and achieve program success while fulfilling core purpose, value, and vision
nConduct initial intake assessments and act upon critical needs appropriately
nOrient clients to the shelter program, requirements and consequences
nProvide client with Independent Living Plan (ILP) (CARES), working in collaboration with other Social Service providers and ensure the delivery of comprehensive support services
nAssist non-working clients to obtain Public Assistance
nCompletes psychosocial assessments within the first 30 days of intake
nTrack and monitor client progress
nEmpower clients to become involved in their own planning and goal setting
nConduct weekly/bi-weekly meetings with clients, or as needed
nRefer clients and encourage job development for subsequent employment if appropriate
nRefer clients to appropriate resources to assist with set goals
nMeet with clients to address issues, challenges, and successes to assist in strengthening their capacity for long-term self-sufficiency
nMaintain client files to include conversations, warnings and progress reports and documenting all incidents
nMaintain confidential case record for all clients
nReport critical incidents immediately to the Site Director/ Director of Social Services
nWork collaboratively with other Housing Specialist and Employment staff to identify client readiness for employment and/or housing
nTreat all clients, visitors, and employees with kindness, respect, and dignity
nReport critical incidents immediately to the Director/Supervisor of Social Services
nRefer clients to internal and external resources and serve as an advocate when necessary
nProvide case management and counseling periodically; one year beyond the client's departure
nDevelop and maintain connections with other programs and units within the organization to facilitate inter-agency referrals
