Case Manager

This position is located in the OKC/Norman area. The Case Manager is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered. The Case Manager will manage a caseload of clients and work to ensure they receive the necessary services and support to achieve optimal health and well-being. Services can take place in the individuals home, community, or in a facility. This role involves assessing clients' needs, developing care plans, coordinating services, and advocating for clients' rights and needs.

Position Specific Duties & Responsibilities:

  • Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
  • Implement individualized care plans in collaboration with clients, families, and other stakeholders that address the unique needs and goals of clients, promoting their reintegration into the community through tailored support and skill-building activities (Varies by Program).
  • Coordinate and monitor the delivery of services to ensure timely and appropriate care.
  • Advocate for clients' rights and assist them in accessing necessary resources and services.
  • Provide emotional and practical support to clients and their families.
  • May regularly evaluate clients' progress and adjust care plans as needed by program.
  • Maintain accurate and up-to-date documentation of clients' care, progress, and outcomes.
  • Facilitate communication and collaboration among multidisciplinary teams, including healthcare providers, social workers, and therapists.
  • Participate in case conferences, team meetings, and other collaborative activities to ensure coordinated care.
  • Educate clients and families about available services and resources and assist them in making informed decisions.
  • Ensure compliance with all relevant laws, regulations, and standards of practice.
  • Participate in continuing education, training, and professional development opportunities to stay current with best practices and trends in case management. Completing certification renewals and all assigned trainings by deadline.

Qualifications:

Education:

Must be a high school graduate or have HS equivalent. Bachelor's degree preferred.

Experience:

Experience in the field of mental health, criminal justice, or community based social services required.

Certifications/Licenses:

ODMHSAS Case Manager II Certification, completed within 60 days of eligibility. Must possess a valid Driver License and satisfactory driving record to use agency and/or personal automobile to travel to locations other than primary office and/or for the transportation of clients.

Other Information:

Safety Sensitive Job Classification: This job is classified as a "safety-sensitive" position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the "safety-sensitive" classification, an employee in this position would be subject to drug and alcohol testing, including random testing. Marijuana is one of the substances included in the drug panel screening. Possession of a medical marijuana license will not excuse you from the testing process or the consequences of testing positive for marijuana per the Family & Children's Services Drug Free Workplace Policy, including possible revocation of a job offer or dismissal from employment. May be required to work evening hours, weekends, and/or holidays.
