About the Role:

Silence Aloud Inc/ The Bridge Wellness Center is looking for a dedicated and compassionate Case Manager to join our growing team in Moreno Valley, CA. This is an exciting opportunity to make a meaningful impact in the lives of individuals by connecting them with the resources and support they need. Monday- Friday 10am-6:30pm Full Time

Responsibilities:

Assess client needs and develop individualized care plans to support their goals and well-being

Coordinate and monitor delivery of services across multiple community and social service agencies

Maintain accurate and up-to-date client records, case notes, and documentation

Serve as a primary point of contact and advocate for clients throughout the case management process

Conduct regular follow-ups and progress evaluations to ensure clients are meeting milestones

Collaborate with internal teams, external providers, and community partners to ensure holistic client support

Identify and connect clients with appropriate community resources, programs, and referrals

Conducts assessments and develops service planning

Requirements:

Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred

Prior experience in case management, social services, or a related role

Strong knowledge of community resources and social service systems in the Moreno Valley/Inland Empire area

Excellent communication, organizational, and interpersonal skills

Ability to manage a diverse caseload and prioritize effectively in a fast-paced environment

Proficiency in case management software and Microsoft Office Suite

Compassionate, client-centered approach with a commitment to equity and inclusion
