Case Manager Position
North Central Behavioral Health Clinic is seeking a Case Manager on the Youth & Family Team in Minot, ND. Be part of the solution to the behavioral health crisis. As a team member for ND Department of Health & Human Services, you will ensure the most vulnerable have access to a high-quality healthcare system in which prevention works, treatment is effective, and people recover. As a Case Manager, you will provide individuals opportunities to manage their own recovery so they can live and work independently in their community. With the support of an interdisciplinary team, you will conduct comprehensive needs assessments, complete, and implement service plans, as well as leverage contacts and referral partners to ensure access to needed services. Case managers also assist with crisis response. Case Management services are community-based. Work will be done primarily in the individual's home and community. To thrive in this position, you should be knowledgeable about resources offered for youth and families in your community and prefer working with the youth in their home and natural environments To be considered for the Case Manager position, you must meet one of the following requirements: a bachelor's or a master's degree or have at least five years of paid supervised experience working with individuals with SMI/SED in a role with case management functions such as individual assessment, care plan development and maintenance, referral and appointment scheduling, monitoring and follow-up activities. This position requires a valid driver's license and successful completion of the assigned defensive driving course during new-hire orientation. Location This position provides essential client services face to face and therefore the person in this position needs to be able to travel to meet clients at their location.
