Case Manager LVN Days
Join our team in Woodland Hills, CA, as a Case Manager LVN for a 13-week contract, working Monday to Friday, 8:00 am to 4:30 pm.
Responsibilities
Manage 20-24 Med Tele patients. Assess discharge readiness and communicate with RNs. Provide discharge education and resources. Identify and resolve discharge barriers. Facilitate patient flow through the hospital.
Requirements
Active CA LVN License. BLS Certification. 2+ years LVN experience in Case Management and Med Tele. Experience in discharge processes. Effective liaison skills between patients, families, and RNs. Travel experience preferred; local travelers welcome.
Additional Information
No on-call duties; floating as needed. COVID vaccine not required. Scrub color: Royal Blue or Navy Blue. PPE provided with on-site fit testing.
