Case Manager

Position: Case Manager

Reports To: Program Manager

What The Case Manager Does
  • List the essential functions/job duties of the role.
  • Maintain professional relationships with clients and adhere to HIPAA regulations.
  • Follow Universal Precautions and Standard Protocols.
  • Communicate effectively with clients, staff and supervisors
  • Maintain a caseload of ten tenants (62 years and older) diagnosed with a serious mental illness (SMI) who were formerly homeless.
  • Maintain and update your assigned charts on daily basis.
  • Maintain at least two monthly tenant contacts in the office and in the apartment.
  • Assist tenants in developing services plans and assessments.
  • Facilitate groups.
Minimum Education/Experience Required
  • Bachelor's degree in Social Work (B.S.W.) or related field and 2 years of relevant experience.
Other Requirements
  • Commitment to the mission and values of CAMBA
  • Knowledge of relevant best practices, regulations and compliance standards required.
  • Strong communication, organizational and multitasking skills.
  • Computer proficiency (MS Office – Word, Excel, Power Point and Outlook).
  • Ability to respond to tenant emergencies or crises and provide acute intervention, de-escalation and or initiate 911 or mobile crisis calls.
  • As a Mandated Reporter, you must immediately report suspected incidents/cases of abuse and neglect to the Vulnerable Persons Central Register (VPCR) Hotline at and complete a CAMBA Incident Report.

Compensation: $46,350 annually When salary ranges are listed, the range would represent the low and high end for the applicable position & program. The salary offered would be based on various factors unique to each program and candidate. This includes but is not limited to experience, education, budget and/or program size, internal equity, skills and other factors that may be required for the position and organization.
