nNexus-Family Healing has an exciting opportunity for a Case Manager to join our team in our Kinship team in our St. Cloud office! This position will work specifically with relative/kinship caregivers.

nOur Case Manager provides support and services to relatives caring for youth in St. Cloud, MN and within the surrounding area. When a relative cares for a child or sibling group removed from their homes, the advantages are tremendous for the children - but the challenges these relatives face can often be large.
nPrimary responsibilities:
n• Networking with community to reach relatives caring for youth either formally or informallynn• Conduct targeted outreach to locate kinship families for youthnn• Connect relative/kinship families with needed services and information while providing direct support to the children and families as needednn• Facilitates share & support groups for relative and kin caregivers.nn• Ensures regular ongoing communication with familiesnn• Complete Home studies and licensing for relative and kin foster care providers.nn• Recognize and value cultural differences in all aspects of work and service deliverynnRequired Education and Licensure:
