Job Description
Provide comprehensive case management services to adult clients , facilitating client advocacy, referral and service linkage via client outreach visits
· Provide individual consultation and facilitation of Wellness & Recovery education groups
· Provide off site client consultation in the family home and or other designated meeting site
· Provided linkage and service coordination to appropriate court ordered treatment modalities.
· Develop extensive knowledge of mental health and social resources in the county, drawing upon team and collateral expertise as needed
· Communicate to team regarding client interpersonal and environmental changes, barriers, and challenges for the development effective treatment planning
· Facilitate groups and individual services to support clients’ treatment goal progress and community integration.
