Case Manager
The Case Manager provides assessment, monitoring, planning, linkage and advocacy for the most appropriate services for individuals and families enrolled in rapid rehousing services so they may access permanent housing and achieve housing stability. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for individuals and families experiencing homelessness.
Essential duties and responsibilities include screening households to determine and document eligibility, assessment of the individual and other family members of their strengths, needs, abilities and preferences to assist in the development of housing goals, developing a Housing Stability Plan, monitoring progress daily and maintaining accurate documentation of progress towards goals and services provided, and providing referrals and facilitating access to services and community resources as needed.
Other responsibilities include complying with all applicable training requirements, company safety, personnel and operational policies and procedures, and contributing positively as a member of a productive and cooperative team.

Required knowledge, skills and abilities include the ability to speak, write and understand English, possess basic computer skills, flexible work schedule including evenings, nights, weekends and holidays, ability to set appropriate limits, work under deadlines and multi-task, ability to organize, prioritize, self-motivate, and deliver results, excellent communication and listening skills, successfully pass law enforcement background screening, valid Florida driver's license if driving an agency vehicle or a personal vehicle for company business, must have reliable transportation, knowledge in harm reduction, motivational interviewing and trauma-informed care, abide by regulatory requirements of all rapid-rehousing funding streams and on the ethical use and application of program's financial policies.
Additional knowledge, skills and abilities include knowledge of homelessness, severe and persistent mental illness and substance abuse, knowledge of resources in the community available for the homeless population, knowledge of wrap around services and providers within CoC, ability to form partnerships in the community and seek out community resources, strong organizational, time management and data management skills, proven ability to work effectively both individually and as part of a team, ability to multi-task and problem solve under pressure, ability to provide customer service to difficult populations, must demonstrate excellent interpersonal skills and possess the ability to interact effectively with other agencies and service providers in order to provide wrap-around services, candidate must be able to work in a fast-paced environment and understand the issues that are faced by low-income populations, ability to make appropriate and time-related services and supports available to families and individuals to allow them to stabilize quickly in permanent housing.
Education and experience requirements include a minimum of a bachelor's degree in social work or related field, minimum 2-years' experience serving homeless or at-risk families and/or individuals in crisis, must be familiar with a multitude of wrap around services and providers within the CoC, must be able to comply with complex governmental regulations, policies and procedures and demonstrate thorough document compliance efforts and activities, must be proficient with data management and information systems and have basic knowledge of Excel, PowerPoint, and Outlook, knowledge of RRH Standards preferred (HPRP, Emergency Shelter Grant (ESG), CoC, Grant Per Diem (GPD), etc.)
General physical demands include prolonged sitting at the computer workstation, standing, bending, reaching, lifting up to 20 lbs. and some driving. Requires manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, telephones, and other office equipment. It is also required to regularly sit, speak, and listen, the employee is also required to walk, use hands and fingers to type, operate equipment, and maintain records and notes. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
Mental demands include handling new and diverse work problems on a daily basis, personal maturity, ability to resolve problems, handle conflict, and make effective decisions under pressure, ability to listen objectively to people, perceive the real problem and assist in bringing issues to a successful conclusion, ability to relate and interact with, volunteers, clients, contractors, visitors and employees at all levels within the Agency.
Work environment includes occasional noise due to equipment operations and interactions among clients and staff, possibility of being exposed to communicable disease, possible exposure to verbal abuse or similar behavior from residents/clients, employees may be called upon to work outside of the established work schedule or work odd hours.
