Case Manager I | Make a Real Impact in Your Community
Location: Sacramento, CA Schedule: Full-Time Pay: $24.90 per hour
Are you passionate about helping others overcome barriers and improve their quality of life? We’re seeking a dedicated Case Manager I to support individuals facing challenges such as housing instability, mental health conditions, substance use, and chronic illness. This is a highly impactful, community-based role where you’ll build meaningful relationships and connect clients to essential resources that change lives.
What You’ll Do
- Manage a caseload of ~50 clients and provide individualized support
- Conduct assessments and develop goal-oriented care plans
- Connect clients to housing, healthcare, income, and community resources
- Provide in-person support (70% field-based, 30% office-based)
- Advocate for clients and coordinate care with partner organizations
- Document services and maintain accurate records in EHR systems
What We’re Looking For
- High School Diploma or GED (Associate’s preferred)
- Experience working with underserved populations (lived experience welcomed)
- Strong communication, organization, and time management skills
- Ability to thrive in a fast-paced, field-based environment
- Valid driver’s license, reliable vehicle, and willingness to transport clients
- CPR/First Aid certification (or willingness to obtain within 90 days)
