Case Manager I - FSP SPA 6
The Case Manager ensures the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Mental Health for contracted agencies as well as adhering to the mission of St. Joseph Center. The position provides case management services (individual rehabilitation, skill building) to adults diagnosed with severe and persistent mental illness and substance use disorders and works within a multi-disciplinary team.
Essential duties and responsibilities include providing case management and supportive services to meet the needs of each individual, building trust and rapport with individuals, providing linkage to physical health, substance abuse, meaningful community activities, volunteer services, job placements etc., providing support with individual rehabilitation and skill building, linking clients to appropriate housing and assisting them in completing required paperwork to obtain appropriate permanent housing, developing and maintaining strong collaborative relationships with other service providers involved in the client's care, collaborating and consulting regularly with FSP team's interdisciplinary team members, conducting ongoing case management assessments identifying client barriers to achieving their individual goals, identifying and utilizing client strengths to increase self-efficacy and independence, providing ongoing intensive case management services that ensure successful permanent housing, decrease social isolation, decrease/prevent relapse risks, advocacy to deter evictions, and linkage and ensure smooth transition into permanent housing by supporting clients with completing benefits and housing application processes, life skills such as budgeting and cooking, accessing community resources, and other skills as needed, providing crisis management such as assessing for suicidal/homicidal ideation when necessary, communicating with landlords and other collaborating agencies, responding to urgent housing requests in a timely manner, participating in meetings with collaborative agencies and other community meetings as directed, and completing and maintaining all documentation necessary by DMH requirements, agency requirements and program requirements.
Qualifications
Knowledge, skills and abilities include being highly motivated and a self-starter, the ability to communicate with and relate to a diverse group of people including clients, community, and other staff, excellent organizational skills and the capability to work in a fast paced environment, strong knowledge of homeless services and resources, demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First, a bilingual background is a plus, and the ability to work flexible hours including some early mornings, evenings.
Experience includes 1+ year of experience providing services to highly vulnerable populations preferred.
Education includes a Bachelor's degree from an accredited college or university in social work, human services or a related field is preferred.
Computer skills include proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and ability to use widely supported internet browsers.
Travel requirements include access to reliable transportation, travel to meet with clients as needed, and travel to meetings at multiple St. Joseph Center sites and at community partner locations as needed.
Work environment characteristics include being able to perform the essential functions of this job in a moderate noise level environment with reasonable accommodations made to enable individuals with disabilities to perform the essential functions.
Physical demands include standing, sitting, walking, stooping, talking, hearing, reaching above and below shoulders, using hand and finger dexterity, keyboarding and making and receiving telephone calls, and lifting and or carrying up to 20 lbs.
