Case ManagerThe Case Manager provides outreach and support to individuals engaged in the re-entry process as well as other justice involved members of the community. They have the clinical and treatment supports necessary for success including medication assisted treatment, therapy, and other community services. Position requires both community support and case management work in the jail, the outpatient clinic, and general community.
The Case Manager assists clients to attain their clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.Responsibilities include:Provide service coordination and linkages to the communityProvide outreach services to clients, delivering mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc.Assist clients in obtaining benefits, housing and health care as neededProvide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plansEncourages and facilitates utilization of clients' natural support systems as well as recovery-oriented, peer support and self-help supports and servicesAssist in data collection and tracking for grant evaluationRequirements:Ability to pass certification for CPR, First AidValid driver's license; reliable transportation for business useAbility to counsel and support clients with psychiatric and/or substance use disorder diagnosesDemonstrated knowledge of trauma-informed careLived experience relative to psychiatric and/or substance use disorder preferred *Recovery Coaches welcome! * Bilingual encouraged to apply: Spanish/EnglishFlexible Hours-Full time or Part timeBenefits:Paid Major HolidaysGenerous Paid Time Off / VacationSick TimeHealth InsuranceDental InsuranceFlexible Spending Accounts (FSA)Health Fitness & Education Class DiscountsEmployee Wellness BenefitsAnd much more!Pay rate ranging between $20-$23/hour based on experience.
