Responsibilities

The ideal candidate will have experience in a medical or rehabilitation setting and be able to address not only clinical needs but also the psychosocial, functional and community reintegration factors that impact recovery and long-term outcomes for a complex neurologic patient population.

The primary job functions in for the Case Management role include:

  Clinical Interventions/Discharge Planning

  Post-acute care planning management

  n

• n

  Payer/Referral Management

  n
n
• n

  Patient and family education and provision of community resources

  n
n

Qualifications

Minimum Qualifications

n
• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
n

Preferred Qualifications

n
• n

  Previous experience in Case Management and Discharge Planning preferred.

  n

• n

  CCM Certification Preferred.

  n
n
