Case Manager (CM) PRN, Inpatient Rehabilitation

Rehabilitation Hospital of Oak Creek is a new 40‑bed inpatient rehabilitation facility, part of Lifepoint Health. We are seeking a Case Manager for a Per Diem schedule, working days only. The role will support patient care and discharge planning in a highly collaborative environment.

How You'll Contribute

• Completes departmental orientation, initial and annual competencies.
• Assists with departmental performance improvement initiatives, collecting and reporting data as requested by supervisor.
• Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care and discharge, and develop solutions.
• Completes documentation per workflow timeline, including Individual Plan of Care per CMS guidelines.
• Schedules family conferences and communicates with caregivers to provide progress updates and goal achievement information.
• Coordinates weekly patient care team conferences to develop, monitor, and refine treatment plans.
• Reviews CMGs for missed comorbid conditions and communicates findings to HIM team.
• Communicates effectively with nursing, therapy, and ancillary departments to ensure proper utilization.

What We're Looking For

• Current Registered Nurse or Social Work licensure or other healthcare professional licensure (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, Occupational Therapist).
• Minimum 2 years social work or case management experience in an inpatient setting (acute/rehabilitation hospital preferred).
• Certifications in Case Management or Rehabilitation Nursing preferred (CCM, ARN, ACM, RN‑BC, etc.).
