Empower HER Foundation is seeking a dedicated Case Manager to provide home-based family services focused on child safety, family stabilization, and prevention of out-of-home placement. This role involves direct engagement with families, case planning, and coordination of services in alignment with DCFS standards.
Key Responsibilities
- Conduct home visits and family assessments
- Develop and implement individualized service plans
- Monitor child safety and family progress
- Coordinate referrals to community resources
- Maintain accurate, timely case documentation
- Participate in weekly supervision and case reviews
- Collaborate with DCFS and community partners
Qualifications
- Bachelor’s degree in Social Work, Psychology, or related field (required)
- 1–3 years of experience in child welfare, case management, or human services preferred
- Strong documentation and organizational skills
- Ability to work independently in community settings
- Reliable transportation required
- Home visiting certification
