Job Description:
Embark on a dynamic travel RN role focused on Case Management, guiding care coordination and discharge planning to optimize patient outcomes. This day-shift assignment in an acute care setting emphasizes collaboration with multidisciplinary teams and includes weekend coverage as needed. A valid state-issued RN license is required, with eligibility for pending licensure accepted so licensure is secured by the start date. Requirements:

• CA state license required
• Must be licensed by start date
• Pending license accepted but must be licensed upon start date
• 2 years acute medical Care Manager experience performing Care Coordination/Discharge Planning
• Weekend requirement: typically one full weekend per 4 weeks
• RTO restrictions: no more than 7 days
• Type of Facility: Acute Care Facility
• First Time Traveler: screened for 1st-time travelers
• Ratios: Days 1:25
