Job Overview
The Care Manager serves as the primary point of contact between members and the health plan. This role is responsible for coordinating care, reviewing person-centered service plans, and ensuring members receive the appropriate covered and non-covered services.
Key Responsibilities
- Conduct monthly member outreach and follow-up
- Address and respond to identified member needs
- Coordinate care with members, families, and vendor agencies
- Maintain timely and accurate documentation
- Participate in weekly team meetings
- Perform additional duties as assigned
