Case Manager

Job Overview

The Care Manager serves as the primary point of contact between members and the health plan. This role is responsible for coordinating care, reviewing person-centered service plans, and ensuring members receive the appropriate covered and non-covered services.

Key Responsibilities

• Conduct monthly member outreach and follow-up
• Address and respond to identified member needs
• Coordinate care with members, families, and vendor agencies
• Maintain timely and accurate documentation
• Participate in weekly team meetings
• Perform additional duties as assigned

RN, LMSW, MSW - bilingual a plus - Chinese, Mandarin, Urdu, Vietnamese, Spanish
