The purpose of the position is to ensure a holistic, whole-person approach to providing patient support, referrals, and resource links. The Case Managers/Coordinator will be responsible for maintaining contact and follow-up care to patients identified. Case Managers/Coordinator will be assigned to a BH location but also be asked to float to other clinics at times.
Work is performed in an office setting and on occasion at arranged job sites as required. Occasional travel will be required in the performance of duties.
Position requires the demands of normal office work and possibly driving a GSA vehicle to arranged job sites as required.
Satisfactory job performance will be determined by successful execution of the following:
- Knowledge of tribal beliefs and customs necessary to courteously and effectively communicate with clients and their families, BHS staff, outside agencies, and the general public both orally and in writing.
- Confers with clients to assess their individual and family needs for appropriate community referrals.
- Develop various community relationships and knowledge of community resources necessary to assist families with identified needs.
- Ensure confidentiality and participant protection procedures are appropriately maintained.
- Confer with staff and BH therapist/consultants regarding client needs
- Regular attendance is required to perform the essential functions of this position.
- Be able to work in an interdisciplinary setting.
- Complete required interagency and other necessary paperwork.
- Willingness to be a team player to ensure success
- Performs other duties as required.
