Social Services – MSW
Day shift, 5 days per week, from 08:00 to 16:30. Weekend requirements include every other weekend and holiday coverage.
1:35 patient ratio. Focus on discharge planning, with responsibilities across up to 2 units on weekends.
2 years of acute hospital case management experience required. Master's degree in Social Work (MSW) preferred. CA state license preferred; pending license accepted. No more than 7 days of RTO restrictions.
Assess and support patients' social needs. Coordinate discharge planning with healthcare teams. Provide counseling and support services to patients and families. Collaborate with community resources to facilitate care transitions. Document and maintain patient records accurately.
