Case Management - Primary Care - Nurse Practitioner

Nurse Practitioner - Direct Hire (PACE Program)
Location: National City, CA
Schedule: Monday - Friday, 8am - 5pm
Must be available to work at multiple sites and participate in an on-call panel, including evenings and weekends.
Position Overview:
We are hiring a dedicated and experienced Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) team. This role focuses on delivering comprehensive care to an elderly population through a collaborative, interdisciplinary approach.
Key Responsibilities:

• Provide primary and preventive care, including diagnosing and managing chronic and acute conditions.
• Collaborate with interdisciplinary teams to support holistic care plans.
• Refer patients to behavioral health professionals as appropriate.
• Supervise nursing staff and support staff in clinical settings.
• Ensure timely and thorough documentation within the EMR.
• Participate in quality improvement efforts and the development of care protocols.
• Provide inpatient and on-call coverage as assigned.

Qualifications:
Education:

• Minimum: Licensed Nurse Practitioner or Licensed Independent Practitioner
• Preferred: Master of Science in Nursing (MSN), National Certification as a Family Nurse Practitioner

Experience:

• Required: Experience in a community clinic and/or managed care setting
• Preferred: Experience working with elderly patients

Certifications:

• Required: RN license and current CPR certification
• Preferred: MSN

Other Requirements:

• Supervisory experience with nurses
• Bilingual (English/Spanish) preferred
• Strong oral and written communication skills
• Proficient in wound care; ACLS certification preferred
