Hospital Case Manager
This is a 13-week contract position.
5x8-Hour Day Shift (08:00 - 16:30) with no weekends or call required. One holiday is required during the contract period.
The Hospital Case Manager will be responsible for the administrative oversight of the overall operations and development of the Care Management Department. This role requires proficiency in hospital case management and discharge planning for adult and geriatric patient populations. The case manager will collaborate with interdisciplinary teams to ensure effective care coordination and compliance with regulatory standards.
Conduct patient assessments and develop care plans in alignment with hospital policies.
Facilitate discharge planning and ensure continuity of care post-discharge.
Review patients for medical necessity and adherence to CMS guidelines.
Coordinate with healthcare providers and resources to optimize patient outcomes.
Document patient interactions and care plans in the EPIC charting system.
Participate in interdisciplinary meetings and contribute to quality improvement initiatives.
Bachelor of Science in Nursing (BSN) required.
Current Basic Life Support (BLS) certification required.
Minimum of 3 years of experience in hospital case management and discharge planning.
Competence with EPIC charting system preferred.
