Smaller community healthcare facility part of a system looking to bring on Case Management Director! Sign On Bonus and Full Relocation!
Responsibilities
- Direct reports include 4 full-time staff and numerous prn staff
- Team consist of all Social Workers
- Typical case load expected of the Case Managers is 11-14 patients
- Director will carry a caseload as needed but should expect weekly to have front line responsibilities.
Qualifications
- Degree required, must possess bachelor’s degree in Nursing (BSN) with RN licensure
- Degreed required in SW, OT, PT, ST, or Rehabilitation Counseling.
- 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning
