Case Facilitator I - Child And Adolescent
A case facilitator I - child and adolescent operates under supervision of the assigned area director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers. Links, monitors, and refers clients to appropriate community resources. Coordinates care with appropriate internal and external parties. Ensures appropriate behavioral health services are provided.
Education: High school diploma or GED, equivalent.
Experience: Must meet OBHP requirements = Two years' experience in mental health or community services preferred.
Licenses/Certification: Valid state issued drivers license.
This is a full-time community based position M-F with a semi-flexible schedule, with a preference of 1-2 evenings until 7pm.
Salary based on experience.
