Care Plan Nurse RN

Position Summary

Fairview Skilled Nursing Home is seeking a compassionate and organized Care Plan Nurse (RN) to coordinate individualized resident care plans, collaborate with the interdisciplinary team, and ensure high-quality, resident-centered care.

Responsibilities

• Develop, update, and maintain individualized care plans.
• Coordinate interdisciplinary care plan meetings.
• Collaborate with nursing, therapy, dietary, social work, and physicians.
• Ensure care plans reflect residents' needs, goals, and regulatory requirements.
• Monitor resident progress and revise care plans as needed.
• Maintain compliance with CMS and New York State regulations.

Qualifications

• Current New York State RN license.
• Long-term care or skilled nursing experience preferred.
• Strong communication, organizational, and documentation skills.
• Ability to work effectively with an interdisciplinary team.
