nJob Summary
nWe are looking for a Care Partner to join our team! You will be directly working with residents, following a one-on-one care plan in the Resident's home. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets the Resident's needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
nResponsibilities
nAssist patients with daily activities, ADLs, such as moving in and out of beds, helping with bathing, getting dressed and getting ready for bed, and in and out of automobiles.
nCare for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
nMaintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
nBuild relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
nGive medications upon completion of training
nEngage patients in exercises or other activities
nQualifications
nGraduated from an accredited Home Health Aide program or we will train thru PA Dept. of Human Services
nHigh School Diploma or GED
nSome prior professional experience
nCPR certification
