Touchstone Communities is seeking a compassionate Care Navigator to guide residents and families through skilled nursing, rehab, and long-term care services. In this role, you’ll coordinate care plans, facilitate transitions between hospital, community, and our centers, and connect families with resources and education. You’ll collaborate with nursing, therapy, and social work teams to remove barriers, support recovery, and enhance quality of life.
