Care Navigator - Licensed Nurse (Fully Remote)

• Build relationships with patients based on trust and motivational communication techniques.
• Triage vital signs and collaborate with care teams to set patient-centric health goals.

Responsibilities:

• Manage physiological markers and patient messaging through a remote care platform.
• Perform monthly wellness calls and provide preventive health education and coaching.
• Meet team goals and follow appropriate escalation pathways for urgent care needs.

What We're Looking For:

• Unrestricted RN/LPN/LVN license in a compact state and ability to work MST hours.
• 1-3 years of clinical experience, ideally with Medicare and remote patient monitoring.
• Strong communication skills and ability to work independently with minimal direction.
