Manager – Care Management (Social Work)Location: Everett, WashingtonCompensation & BenefitsSalary range: approximately $110,000 – $175,000 annually, depending on experienceFull-time leadership positionComprehensive benefits packageRelocation assistance may be availableAbout the OpportunityStar Nursing is seeking an experienced Care Management Manager – Social Work to oversee hospital-based care coordination and case management services.This leadership role manages care management operations including social work services, discharge planning, utilization management, and patient care coordination. The Manager works closely with physicians, clinical teams, and community resources to ensure efficient care transitions and high-quality patient outcomes.Position SummaryThe Care Management Manager provides leadership and oversight for the Care Management Department, ensuring best practices in care coordination, discharge planning, and resource utilization. This role collaborates with interdisciplinary healthcare teams to maintain continuity of care while supporting organizational goals related to patient outcomes and operational efficiency.Key ResponsibilitiesCare Management LeadershipManage daily operations of care coordination, case management, discharge planning, and social work servicesLead care management staff to ensure high-quality patient-centered careImplement and monitor clinical best practices and departmental standardsOperational OversightOversee utilization management and pre-admission case management activitiesMonitor departmental performance and quality metricsEnsure compliance with regulatory and organizational standardsCollaboration & CommunicationServe as liaison between patients, families, physicians, and healthcare teamsCoordinate with community agencies, skilled nursing facilities, and external partnersSupport smooth care transitions and discharge planningTeam Leadership & DevelopmentRecruit, train, and supervise care management and social work staffSupport professional development and performance improvementPromote a collaborative, patient-centered team environmentMinimum QualificationsMaster’s Degree in Social Work (MSW) from an accredited programWashington Clinical Independent Social Worker (LICSW) licenseMinimum 6 years case management experience in an acute care settingMinimum 2 years leadership experience at Manager or Director levelExperience working in hospital-based care coordination programsPreferred QualificationsLeadership experience in large hospital environmentsStrong knowledge of utilization management and discharge planningExperience managing multidisciplinary care management teamsWhat You Can ExpectLeadership role impacting patient care coordinationCollaborative hospital environmentOpportunity to improve care transitions and patient outcomes
