The Manager works closely with physicians, clinical teams, and community resources to ensure efficient care transitions and high-quality patient outcomes.Position SummaryThe Care Management Manager provides leadership and oversight for the Care Management Department, ensuring best practices in care coordination, discharge planning, and resource utilization.
