Care Manager (RN)

Location: Sacramento, open to out‑of‑state candidates – relocation bonus provided

Position: Full‑time

Earn a $5,000 Bonus! $2,500 after 90‑day training completion + $2,500 at 6 months.

Role Scope: As the Care Manager (RN) you will be responsible for the management and delivery of direct nursing care to our participants in a variety of settings, including the clinic, adult day center, participants’ homes, virtual care, and SNF. You will serve as a critical member of the Interdisciplinary Team (IDT) and act as the “quarterback” for your panel, working collaboratively to complete assessments and advance participant care plans.

  • Participate and facilitate IDT meetings, contributing insights from assessments and care plan recommendations and coordinating care in a collaborative spirit.
  • Conduct face‑to‑face nursing assessments of physical, psychosocial, and behavioral status in various settings, primarily at the Habitat center and occasionally in‑home.
  • In partnership with a medical provider, deliver personalized care for a panel of participants based on care plans.
  • Deliver and document nursing interventions as agreed upon in participants’ care plans, respond promptly and accurately to physician orders, and correctly administer medications and therapeutic interventions.
  • Provide longitudinal case management and coordinate transitions of care, including complex patient discharges, transfers, and immediate post‑discharge needs with hospital and long‑term care facility case managers.
  • Coordinate all aspects of care delivery including medication management, medical equipment and supplies, and specialist and diagnostic referrals.
  • Triage in the outpatient setting, initiating therapies within scope of practice and collaborating with a medical provider to escalation care as needed.
  • Educate participants, caregivers (including family members), and team members on personalizing and carrying out care plans.
  • Perform wound care (including complex wounds), IV therapy, and other procedures within RN scope of practice to meet evolving participant needs.
  • Delegate tasks to MA and Licensed Vocational Nurses within their respective scopes of practice.
  • Remotely take after‑hour calls that are triaged on a rotating schedule.
  • Perform related duties as assigned.

Required Qualifications:

  • Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred.
  • Unencumbered California Registered Nurse (RN) license required.
  • Minimum 2–4 years of clinical experience caring for formally complex or older adult populations as an RN.
  • Minimum 1–2 years of case‑management experience.
  • Strong clinical acumen in chronic disease management and complex geriatric care.
  • Experience in managing clinical interventions: wound care, IVs, phlebotomy, colostomy/ileostomy care, etc.
  • Proof of medical clearance for communicable diseases, including a TB test.
  • Proof that all immunizations are up to date.
  • Proof of current CPR/BLS certification, or commitment to obtain within 30 days of employment.
  • Proof of a valid California driver’s license, personal transportation, good driving record, and auto insurance as required by state law.

Preferred Qualifications:

  • Alzheimer’s certification preferred.
  • Case Management certification preferred.

Compensation:

The anticipated base salary range for this position is $55.00–$73.00 per hour. Final compensation will be based on qualifications, skills, experience, education, certifications, geographic location, and business needs.
