Care Manager (RN) - Must Live in Illinois

About This Role

Molina Healthcare is seeking an Illinois licensed RN to join as a Care Manager, focusing on Medicare members. This role involves completing assessments and coordinating care services, primarily through telephonic communication, with occasional face-to-face meetings.

• Schedule: Full Time, Monday – Friday, 8 AM – 5 PM CST

What You'll Do

• Complete comprehensive assessments of members per regulated timelines.
• Develop and implement care coordination plans with members and healthcare professionals.
• Conduct telephonic, face-to-face, or home visits as required.
• Perform ongoing monitoring of care plans to evaluate effectiveness.
• Maintain an ongoing member caseload for regular outreach and management.
• Promote integration of services for members, including behavioral health and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and support members.
• Assess for barriers to care and provide assistance to members.
• Conduct medication reconciliation as needed.

Requirements

Education

Registered Nurse (RN) License must be active and unrestricted in state of practice.

Experience

At least 2 years experience in healthcare, preferably in care management.

Licenses & Certifications

Valid and unrestricted driver's license.

Understanding of electronic medical records (EMR) and HIPAA.
