What You'll Do
- Manage complex cases and serve as a liaison between acute care and critical care teams.
- Coordinate efforts of social workers, feeding specialists, PTs, OTs, cardiologists, intensivists, and neonatologists.
- Direct all aspects of inpatient care, ensuring optimal length of stay.
- Anticipate outpatient service needs, setting appointments and consultations prior to discharge.
Requirements
Education
- Bachelor's degree in Nursing (BSN) required
Experience
- 3 to 5 years of healthcare experience in an acute care setting
- 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting preferred
- 1 year of leadership experience in a healthcare setting preferred
Licenses & Certifications
- Current NYS Registered Nurse (RN) licensure required
