Care Manager

About This Role

The Care Manager develops, assesses, and facilitates complex care management activities primarily for members with mental and behavioral health needs, aiming to provide high-quality, cost-effective healthcare outcomes.

• Schedule: Full Time, Day shift

What You'll Do

• Evaluate member needs via phone or in-home visits and recommend/facilitate care plans
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs
• Develop ongoing care plans for members with high acuity and identify necessary providers and resources
• Coordinate between members, families, community resources, and care provider teams
• Monitor care plans and member status for changes in treatment and provide recommendations
• Facilitate care coordination and collaborate with providers to ensure timely access to services
• Collect, document, and maintain member information to ensure compliance with regulations
• Provide education to members and families on healthcare options and benefits
• Provide feedback to leadership on opportunities to improve care delivery
• Perform other duties as assigned

Requirements

Education

• Master's degree in Behavioral Health or Social Work, or Degree from an Accredited School of Nursing

Experience

• 2 – 4 years of related experience

Licenses & Certifications

• One of the following licenses required:
  – Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC), OR
  – RN based on state contract requirements with BH experience
